This form and $400 deposit (check made payable to WITTE TRAVEL) is due SEPTEMBER 16, 2008.

Credit Cards will not be accepted by Witte Travel & Tours for the deposit or any portion of the tour cost. For other credit card options, please contact the tour coordinator, Julie Hale at southafrica2009@q.com

Passport Information

U.S. citizens are required to have a valid passport. Your passport is required to be valid for at least six months beyond the dates of your trip (February 2010). Please check your expiration date and give yourself plenty of time to renew if necessary.
All non-U.S. citizens should contact Witte Travel & Tours for their special requirements. They will check the requirements for non-U.S. citizens both at the time they register for the tour and again approximately 45 days prior to departure. Witte Travel & Tours is not responsible for any changes in requirements for non-U.S. citizens that occur after our final check.

VISAS are not required for travel to South Africa.

PASSPORT VISA PAGES:
When you travel to South Africa, you will need to be sure to have AT LEAST TWO BLANK PAGES in your passport. This does not include the last two pages which are for endorsements and amendments. Trip Insurance

It is recommended that you get traveler’s insurance. Witte Travel includes a medical protection plan in the cost of your trip. They also recommend that each traveler purchase a complete travel protection package and they offer such a plan provided by Trip Mate. Read about the included MEDICAL PROTECTION PLAN and the TRAVEL PROTECTION PLAN you can purchase from them in their TERMS AND CONDITIONS document.

Travel insurance may also be purchased independently. There are a lot of travel insurance companies available. Here are some helpful links to get you started.
